Do I need a referral?

Yes.  Dr. Merifield requires a provider’s referral for all new patients.   Please ask your provider to fax recent chart notes, imaging reports, and demographics to our office, fax number (360) 692-2329.  Once we receive your referral it will be reviewed by Dr. Merifield and our office will contact you.

What can I expect at my first appointment?

As a new patient, your first visit will be a 30-45 minute consultation with Dr. Merifield.  Your diagnosis and treatment options will be discussed.  Procedures, imaging, and/or referrals will be scheduled at this appointment as indicated. 

Please be sure to bring your completed new patient paperwork, including a complete list of your medications, to your consultation.  You will also want to bring all of your insurance information.

Where are procedures preformed?

Dr. Merifield utilizes the Surgery Center of Silverdale, The Pacific Surgery Center in Poulsbo, Harrison Medical Center in Silverdale, and the Sequim Same Day Surgery Center.  Please select Facilities for more detailed information.
